Predictors of Mechanical Ventilation in Guillain–Barré Syndrome with Axonal Subtypes

Background:
The early clinical predictors of respiratory failure in Latin Americans with Guillain–Barré syndrome (GBS) have scarcely been studied. This is of particular importance since Latin America has a high frequency of axonal GBS variants that may imply a worse prognosis.
Methods:We studied 86 Mexican patients with GBS admitted to the Instituto Nacional de Ciencias Médicas y Nutrición Salvador Zubirán, a referral center of Mexico City, to describe predictors of invasive mechanical ventilation (IMV).
Results:The median age was 40 years (interquartile range: 26–53.5), with 60.5% men (male-to-female ratio: 1.53). Most patients (65%) had an infectious antecedent (40.6% gastrointestinal). At admission, 38% of patients had a Medical Research Council (MRC) sum score <30. Axonal subtypes predominated (60.5%), with acute motor axonal neuropathy being the most prevalent (34.9%), followed by acute inflammatory demyelinating polyneuropathy (32.6%), acute motor sensory axonal neuropathy (AMSAN) (25.6%), and Fisher syndrome (7%). Notably, 15.1% had onset in upper limbs, 75.6% dysautonomia, and 73.3% pain. In all, 86% received either IVIg (9.3%) or plasma exchange (74.4%). IMV was required in 39.5% patients (72.7% in AMSAN). A multivariate model without including published prognostic scores yielded the time since onset to admission <15 days, axonal variants, MRC sum score <30, and bulbar weakness as independent predictors of IMV. The model including grading scales yielded lower limbs onset, Erasmus GBS respiratory insufficiency score (EGRIS) >4, and dysautonomia as predictors.
Conclusion:These results suggest that EGRIS is a good prognosticator of IMV in GBS patients with a predominance of axonal electrophysiological subtypes, but other early clinical data should also be considered.
The evolution of a tropical rainforest/grassland mosaic in southeastern Brazil since 28,000 14C yr BP based on carbon isotopes and pollen records

The lack of paleoecological records from the montane Atlantic Rainforest of coastal Brazil, a hotspot of biological diversity, has been a major obstacle to our understanding of the vegetational changes since the last glacial cycle. We present carbon isotope and pollen records to assess the impact of the glaciation on the native vegetation of the Serra do Mar rainforest in São Paulo, Brazil. From ca. 28,000 to ∼ 22,000 14C yr BP, a subtropical forest with conifer trees is indicative of cool and humid conditions. In agreement carbon isotopic data on soil organic matter suggest the presence of C3 plants and perhaps C4 plants from ∼ 28,000 to ∼ 19,000 14C yr BP. The significant increase in the sedimentation rate and algal spores from ∼ 19,450 to ∼ 19,000 14C yr BP indicates increasing humidity, associated to an erosion process between ∼ 19,000 and ∼ 15,600 14C yr BP. From ∼ 15,600 14C yr BP to present there is a substantial increase in arboreal elements and herbs, indicating more humid and warmer climate. From ∼ 19,000 to ∼ 1000 14C yr BP, δ13C values indicated the predominance of C3 plants. These results are in agreement with studies in speleothems of caves, which suggest humid conditions during the last glacial maximum.
Influence of Heat Treatments on a NiTi Shape Memory Alloy Obtained Using Vacuum Induction Melting and Reprocessed by Plasma Skull Push-Pull

Shape memory alloys (SMA) are metallic attractive engineering materials due to their capacity to store pre-defined shapes through a thermally induced phase transition from a solid state. This paper aims to evaluate the influence of solubilization thermal treatments on a NiTi shape memory alloy originally fabricated by vacuum induction melting and then reprocessed by plasma melting followed by injection molding (Plasma Skull Push Pull process) into different metal molds (steel, aluminum, brass and copper) in order to compare the thermal properties regarding to its raw state. The thermal treatments of solubilization were carried out at 850°C in different times (2n function, n = 0, 1, 2 and 3, in hours). The influence of solubilizing treatments in the NiTi shape memory alloy was analyzed using the following characterization techniques: Differential Scanning Calorimetry (DSC) and Electrical Resistance as a function of Temperature (ERT). The results demonstrate that the solubilization heat treatments applied on the reprocessed NiTi shape memory alloy through the plasma skull push pull process, provides important changes in the phase transformation of the material. Therefore, it was demonstrated that it is necessary to solubilize the material after melting or remelting the NiTi shape memory alloy via this process to obtain mini-actuators products with homogeneous properties.

Shape Memory Alloys (SMA) metallic materials that change their mechanical and physical properties with temperature variation and mechanical loading, surprising engineers and researchers. In this way, one can develop thermomechanical actuators capable, for example, of generating force by blocking the shape recovery or change the natural frequency of a mechanical system by blocking resonance. The processing of these SMA are countless, each one with its specific limitation and particularity. This study aims to evaluate the influence of rapid solidification of a Ni-Ti SMA that is originally manufactured by Vacuum Induction Melting (VIM) and reprocessed by Plasma Melting (PM) followed by injection molding into different metal molds (steel, brass, aluminum and copper). The influence of such a processing is analyzed through Differential Scanning Calorimetry (DSC) and Electrical Resistance as a function of Temperature (ERT) to determine the effects on transformation temperatures. The results demonstrate that by using the copper mold one can provide greater uniformity of the material properties. Thus, there is the possibility of obtaining different kinds of SMA mini-actuators by PM injection in a copper mold and that includes different shapes and sizes that can be studied further.

The role of activin-A in follicular development and on the mRNA expression levels of different genes in goat secondary follicles was evaluated. Goat secondary follicles (≥150 μm) were cultured for 18 days under control conditions or with the addition of either 50 or 100 ng/ml activin-A (Experiment 1). The mRNA levels for the genes that code for activin-A, ActR-IA, ActR-IB, ActR-IIA, ActR-IIB, follicle stimulating hormone receptor (FSH-R) and P450 aromatase were measured in each condition (Experiment 2). We observed that after 6 days of culture, the antrum formation rate was higher in cultures with added activin-A than in the cultured control (P < 0.05). The addition of 50 ng/ml activin-A increased the follicular growth rate in the final third of the culture (days 12–18), resulting in a higher percentage of meiosis resumption (P < 0.05). On day 6, the addition of activin-A (50 ng/ml) increased the levels of ActR-IA mRNA compared with the cultured control (P < 0.05). After 18 days, the addition of 50 ng/ml activin-A significantly increased the levels of its own mRNA compared with the non-cultured control. Moreover, this treatment reduced the mRNA levels of P450 aromatase in comparison with the cultured control (P < 0.05). Higher levels of P450 aromatase mRNA were found for both activin-A treatments compared with the non-cultured control (P < 0.05). No difference in estradiol levels was detected among any of the tested treatments. In conclusion, the addition of activin-A to culture medium stimulated early antrum formation as well as an increase in the daily follicular growth rate and the percentage of meiosis resumption.
Review of the distribution and conservation status of the terrestrial reptiles of the Cape Verde Islands

Cape Verde has a higher number of reptile taxa and endemics than any of the five archipelagos in the Macaronesian region. Mapping the precise distributions and assessing the conservation status of reptiles is the first step towards effective conservation. Presence/absence and abundance data were gathered from extensive fieldwork and post-1980 literature. Evaluation of conservation status was considered at specific and subspecific levels, following IUCN Red List criteria and using RAMAS. Fieldwork confirmed the occurrence of 34 of 37 previously recorded taxa (31 native, three exotic). One taxon continues to be considered Extinct. Three broad distribution and rarity patterns were identified: widespread and abundant taxa occurring on ≥ 2 islands/islets, widespread or abundant taxa restricted to one island, and rare or limited range taxa occurring on small areas of islands or islets. More than a third of taxa have areas of occupancy < 20 km2 and extents of occurrence < 100 km2. Geckos are rarer than skinks because of their high habitat specialization, with 58% occurring on only one island/islet. About half of all taxa are potentially threatened, twice the proportion of those in the Canary Islands, a difference that could be explained by the smaller area and greater aridity of the Cape Verde islands. The criterion used for most threat categorizations is geographical range, and the most pervasive threats are natural disasters, intrinsic factors of the species and introduced species. The importance of applying conservation status at the subspecific level to island endemics is emphasized. Several conservation measures are proposed, including optimized design of protected areas.

An in situ tagging technique for the long-spined sea urchin Diadema aff. antillarum, suitable for carrying out short and medium term ecological studies is described. It is a simple method in which external nylon tags are anchored in holes drilled through the urchin test. The technique was tested in the field and found to be effective for individuals of more than 20 mm test diameter in terms of survival and tag retention rates.
